PM Surakshit Matritva Yojana Application Form
Government has issued an application form to claim benefits of Pradhan Mantri Surakshit Matritva Bandhan Scheme, The application duly filled should be submitted to the Aanganwadi workers nearby. Then supervisor will examine the details of form and then submit the form to Child Development Officer. The verified application form will then be submitted to Social Welfare Department of that district. After that the form will be submitted to Central Government for grant of aid.
The central government will then transfer the amount directly to the beneficiary bank account. The linking of Aadhaar Card of both parents is necessary to avail the benefits of Surakshit Matritva Yojana. If the Aadhaar card is not available a declaration letter is needed in which it is written that beneficiary will soon apply for Aadhaar card and present it to the authority as soon as possible.
How to Avail Benefits Under Surakshit Matritva bandhan Yojana
First of all pregnant women have to undergo pregnancy test, the reports of this test must be submitted along with the application form. Once the pregnant women is registered and the details provided by her are verified an amount of Rs 2000 will be transferred to her account. For second installment the test is to be conducted by the hospital after receiving the certificate another sum of Rs 2000 is transferred. The last installment of Rs 2000 is transferred after the birth of child, but applicant has to submit proofs of BCG, OPV, DTP and Hepatitis B vaccination.
